TIPS FOR SENDING A CASE:

You may write a detailed description, or a very short note, as you prefer.

It is recommended that you state at least the drug you took, for how long, and your age if possible.

Please take into account that if you do not discern between information that has to be kept confidential and information that can be published, it is assumed that the whole content that you send will be published. Therefore, you may keep the report completely anonymous or for example include a personal contact address if you like that others might communicate with you, it is entirely up to you.

You can send modifications of your case, updates or a request for deleting it, at any time.
